Summary

This presentation will focus on two initiatives which have successfully transformed clinical and organizational practices in primary care through the use of PREMs/PROMs. The LORI project from Quebec is a research-enabled platform combined with a continuing education program that integrates PROMs and PREMs into primary care. It supports the capacity of clinical and managerial team members to understand and effectively use PROMs and PREMs, while also enabling the generation of patient-reported data for health services research, quality improvement, and goal-oriented care. The EQ-5D PROMs tool, piloted by the Ontario based Alliance for Healthier Communities in 2022, was integrated in a subset of Ontario Community Health Centres (CHCs). This session will cover the results of the pilot project as well as how these results were used to spread and scale adoption across the CHC sector using the Rapid Action Learning Intensive-PROMs framework.

 Objectives

  1. To demonstrate how patient-reported outcome and experience measures (PROMs and PREMs) can be used to transform clinical and organizational practices in primary care.
  2. To highlight the LORI project as a concrete platform offering tools and implementation support to help primary care teams collect, analyze, and use patient-reported data to improve care quality and alignment with patients’ needs
  3. To understand how the EQ-5D PROMs tool was implemented to support shared decision-making and collaborative care within Ontario CHCs.
